A beautiful curling strike by Gaston Pereiro gave Cagliari a deserved lead on the hour-mark with David Ospina to blame for his sloppy reaction.
Napoli responded with three minutes left as Mario Rui delivered a perfect cross for Victor Osimhen to head in the equalizer.
The draw leaves Napoli third in the table, level on 54 points with Inter who have a game in hand. Cagliari stay 18th on 22 points.
